I understand Shapes by Lou does not provide medical advice, diagnosis or treatment, and that services do not replace care from my surgeon or doctor. I will tell Lou about any changes in my health, symptoms or surgeon’s instructions, and I understand a session may be postponed if needed. I understand the $100 deposit is non-refundable.
